Lift collects personal information about our patients such as personal details, medical records, family information employment demographic and account data. This information enables our clinical staff to properly assess, diagnose, treat and be proactive in your health care needs.

Wherever possible Lift will collect your personal information directly from you or a person authorised by you (e.g. relative, guardian or other health care provider). Your personal information will only be used for the purposes for which it collected, or otherwise permitted by law.

Patient medical records are the notes made by the clinical staff as part of the process of providing health care. All patient records are therefore the property of Lift Cancer Care Services. By attending an appointment at Lift, you have implicitly given the clinical staff permission to take notes of the consultation. However, under the Privacy Act 1998 (Cth) you have the right to view and access a copy of your records.

Requests to access personal health information should be directed to the Lift Practice Manager. You will be asked to complete a form that documents your request in writing.

Yes. Under the Privacy Act 1998 (Cth), Lift can charge a reasonable fee for the administrative tasks of providing a copy of your medical records.

Yes. While there is no legal obligation for a Clinician to transfer patient medical records, the Clinicians at Lift Cancer Care Services honour the professional courtsey. This is done by the Clinician at the new practice submitting a formal request to Lift to transfer medical records on behalf of you, the patient. You will be asked to sign a consent form authorising the request.

The management of Lift Cancer Care Services are committed to protecting the privacy of our patients within our practice. Information collected is kept strictly confidential and used only for the medical and health care of patients.
Lift Cancer Care Services is bound by the Australian Privacy Principles under the Privacy Act 1988 (Cth) and other relevant laws about how private health service providers handle personal information and applies to all employees and patients of Lift Cancer Care Services.
We are committed to complying with all applicable privacy laws which govern how Lift Cancer Care Services collect, uses, discloses and stores personal information.

COMPLAINTS ABOUT PRIVACY
The management of Lift Cancer Care Services understands the importance of confidentiality and discretion with the way we manage and maintain the personal information of our patients.
The Centre takes complaints and concerns about the privacy of patient’s personal information seriously. Patients should express any privacy concerns in writing. The Centre will then attempt to resolve it in accordance with its complaint resolution process.
All employees at our Centre are required to observe the obligations of confidentiality in the course of their employment and are required to sign Confidentiality Agreements.
In the instance where you are dissatisfied with the level of service regarding your privacy, we encourage you to discuss any concerns relating to the privacy of your information with the CEO.

PRIVACY STATEMENT
Lift Cancer Care Services will collect your personal information for the purpose of providing you with health care and for directly related purposes.
For example, we may collect, use or disclose personal information:
· For use by a multidisciplinary treating team;
· To liaise with health professionals, Medicare or your health fund;
· In an emergency where your life is at risk and you cannot consent;
· In the management of our hospital, including to risk management, safety and security activities and quality assurance and;
· For the education of health care workers;
· To maintain medical records as required under our policies and by law; or
· For other purposes required or permitted by law.
